Posttest/Evaluation

CME certificate requests cannot be processed without the posttest and evaluation form. Participants must receive a score of 70% or better to receive credit. Materials must be received by June 30, 2009. After this date the activity will no longer be designated for credit. A CME certificate will be mailed within 6-8 weeks. It is recommended that you keep a copy of your completed materials until you receive your certificate. Please note that you may be contacted regarding a post activity survey.

Complete the posttest online at www.cmeuniversity.com. Click on "Find Post-tests/Evaluation by Course" on the navigation menu, and search by project ID 5297. Upon successfully completing the posttest and evaluation, your certificate will be made available immediately.
